I didn't quite get it but my mate (a fan of the games) explained that Silent Hill can be a state of mind, so when they were in their house but it was still foggy, yes they were still in Silent Hill because of their mental state.
Don't know if this is true, but it works for me!

You got the point here. Characters are seem to be trapped in Alesa's living nightmare to surreal to be considered physical. On the other hand Rose, Sheron and Cybil may have died in car accident at the Silent Hill town and that's how they became trapped there spiritually. Silent Hill I think is somekind of purgatory where that nightmarish un-reality and protagonist's state of mind seem to collide. Just thoughts...
